FORMS AND FEES
Please use the forms below for all MHF Inc. transactions.
Junior Horses up to 48months of age $15.00
Geldings all ages $10.00
Mares 48 months of age and over upgrade $20.00
Stallions 48 months of age and over upgrade $20 (DNA required)
FORMS
Work Order - must accompany all paperwork
Membership Application Form - for new members
Membership Renewal Form - For members renewing their membership
(membership expires 31st December each year)
Registration Form - Form used for registering horses that are new to MHF
Registration Update Form - This form covers Senior upgrade, height upgrade and any changes to horse info, colour etc.,
Transfer/lease Form - Form to change ownership of horse or advise horse being leased.
Service Certificate - To be used when an MHF registered stallion serves a mare (regardless of which association the mare is registered with) the owner of the MHF registered stallion must supply the owner of the mare with a service certificate.
Certificate of Castration - Form to be used to advise of colt/stallion being gelded.
Veterinary Certificate of Soundness - This form needs to be completed for MHF registered stallions that are 36months and over unless:-

The adult stallion has already passed a VCS with another recognised miniature horse association

Proof can be provided to show SVSC was carried out and passed (permanent adult stallion papers)
